Service Coordinator

Ultimate Staffing's client, a highly respected non-profit organization, is hiring Direct Hire Service Coordinators for locations in Baltimore and Essex to provide essential support and case management services for residents in a senior affordable housing community. This position is critical in helping residents maintain independent living while enhancing their quality of life. You will collaborate closely with residents, advisory councils, and community stakeholders to proactively address needs, connect residents to key resources, and foster meaningful connections that promote well-being.

Responsibilities

As a Service Coordinator, you'll oversee and coordinate support services to empower residents to live independently and thrive. Your responsibilities will include:

  • Partnering with residents, council members, and committees within the community to collaboratively address their needs.
  • Conducting comprehensive assessments for at-risk or elderly residents on a voluntary basis to evaluate their needs and develop appropriate plans of action.
  • Facilitating the smooth transition of incoming residents by identifying and addressing service or support needs.
  • Collaborating with external providers to coordinate general case management services and make referrals as necessary.
  • Maintaining detailed and accurate documentation using an online case management system, including resident files, activity logs, and program records.
  • Establishing and maintaining relationships with local service providers, ensuring quality service delivery and adherence to established vetting standards.
  • Organizing and leading educational workshops and programs to support health, social engagement, and personal well-being.
  • Encouraging residents to maintain and expand informal support systems, including family, friends, and their local community.
  • Supervising social work interns (e.g., BSW, MSW) and other trainees, as appropriate.
  • Continuously improving the quality of programs and services through training, research, and evaluation of outcomes.
  • Participating actively in team meetings and reporting progress to leadership.

Qualifications and Requirements

We're looking for a compassionate and self-driven individual with the following qualifications:

  • A Bachelor's degree in Social Work, Human Services, or a related field.
  • 2-3+ years of experience working with older adults in a community-based setting preferred.
  • Familiarity with senior services, aging community needs, and related resources to support independent living.
  • Proven ability to balance autonomous work with team collaboration to meet the expectations of the HUD Service Coordinator Program.
  • Proficiency in Microsoft Office Suite (including Word, Excel, and Publisher) and the ability to manage case files and databases accurately.
  • Strong organizational and multitasking skills to manage priorities effectively.
  • Outstanding written and verbal communication abilities to effectively interact with stakeholders.
  • Creative problem-solving skills with the ability to implement innovative solutions.
  • Passion for advancing equity, inclusion, and diversity within systems and communities.

Compensation

  • This is a direct hire position paying 45-48k
  • Once hired the employee will be eligible for full benefits
